Performing 3L5W Analysis

The fishbone or Ishikawa diagram is one way to show cause-effects (a.k.a., root cause analysis-RCA).
Another method uses a 3L5W template.

Three-Legged Five Why (3L5W) Template in QI Macros Add-in for Excel

To open the template, click on QI Macros, then Improvement Tools and the 3 Legged 5 Whys template is at the top right under the Fishbone.

In the worksheet tab labeled 3L5W Outline:

  1. Enter the problem statement in Cell B2 for the issue (see below)
  2. Describe the Specific Problem in A2
  3. Ask Five Whys to complete. Check logic backwards to avoid issues (therefore). And then clear any cells with unused whys.
  4. The Root Cause (last why in the list) of the Specific Problem becomes the Detection Failure problem (A6).
  5. Ask Five Whys to find root cause of detection failures. Check logic backwards to avoid issues (therefore). And then clear any cells with unused whys.
  6. The Root Cause of the Detection Failure becomes the Systemic Failure problem (A10).
  7. Ask Five Whys to find root cause of systemic failures. Check logic backwards to avoid issues (therefore). And then clear any cells with unused whys.
