Greenbelt Project Checklist
Here's what I consider to be the key requirements for a Green Belt Project (defects or deviation). All charts should be drawn with the QI Macros Lean Six Sigma SPC Software for Excel.

	Defects	Deviation (Variation)
	Project began with data about the problem (defects or deviation), not by trying to fit a Six Sigma project to some accidental, knee-jerk success.
	PivotTable of defects from a database (optional)
	Analyze defects over time	Analyze deviation over time
	Count of Defects/Costs	Variation using Control Chart data
	Analyze Root Causes Head of the Fish is the "Big Bar" on the Pareto Chart	Analyze Root Causes of Deviation Use for special (out of control points) and common causes.
	Countermeasures and Action Plan	Countermeasures and Action Plan
	Countermeasures describe clear methods for changing the process to prevent defects and deviation, not detect or correct them after they occur. Countermeasures should not involve: adding more people (especially inspectors) spending more money automating the detection and correction process solving the problem with training.
	Verify Results by comparing before and after control chart showing improvement	Verify Results by comparing before and after control chart showing improvement
